Targeted Case Manager 1
TCM 1 is an entry level position and requires close to moderate supervision. The TCM 1 is responsible for providing a wide variety of services and supports to the individuals we serve to help them to maximize their quality of life. The position provides case management services to a group of consumers living in various community living arrangement programs or their own homes throughout the county. This includes initial intake interviews, appropriate referrals, liability information and completion of progress notes, consumer contacts as well as ongoing services such as coordination and monitoring.
Requirements: 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
Starting Salary: $39,381
Targeted Case Manager 2
TCM 2 has the same responsibilities as TCM 1, with less supervision.
Requirements: six months of experience as TCM 1, a bachelor’s degree with a social welfare major, or any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
Starting Salary: $42,096
Targeted Case Manager 3
TCM 3 has the same duties as TCM 1 & TCM 2, but with additional team leader responsibilities. On a systemic level, this position supports supervisory staff in the coordination, tracking, and monitoring of CMP MH/DS Initiatives.
Requirement: two years of experience as a TCM 2.
Starting Salary: $44,812
